JOB VACANCY NOTICE POSITION:
Team Leader (Full-Time) Galleton Commons and Supportive Housing Galleton Commons is an enhanced supportive housing program designed to create an atmosphere of security, comfort, and recovery, all within the least restrictive environment.
Staff are onsite 24/7 and are always mobile throughout the grounds 24 hours of the day and available to the clients to provide oversight, support, and assistance with transportation to appointments, meal preparation, general housing keeping, and basic daily needs.
A Team Leader will provide guidance, instruction, direction, and leadership of staff to achieve goals that contribute to the growth of the program.
Displays excellent interpersonal skills to build quality relationships with the team to coordinate and manage the day-to-day activities and required services to all residents/clients.
In your role as a Team Leader, you will also assist the Program Supervisor with the billable case management activities and the coordination of staff to ensure the provision of all recommended and required services.
In addition, as a Team Leader you will ensure staff gives support that follows individuals care plans and goals, work with the supervisor to create monthly staff schedules and monthly resident activity schedules.
Also, as a Team Leader you will provide direct care and support to residents, as necessary and participate in staff development by providing support to enhance skills and knowledge base of case management theory and supervisory techniques.

Qualifications:
Master's degree in social work or other related social service-related field or a bachelor's degree in Social Work or related field, plus 2 years of mental health direct care experience, or High School Diploma with 10 years of mental health direct care experience, plus two years in a supervisory role.
Demonstrated knowledge of governmental program regulations and policies, and other social service providers available to eligible clients.
Demonstrated ability to make incisive observations and obtain vital information during the client/ family interview and/or while reviewing case records to make appropriate recommendations.
Demonstrated ability to establish and maintain effective working relationships with internal staff, providers, and other social service agencies.
Must successfully obtain Criminal History Clearance from PA State Police, Child Abuse Clearance from Department of Human Services, and FBI Fingerprint Clearances.
Must not be excluded from participation in Medicare, Medicaid, or other federal health care program.
